Treatment of Pathologic Perforator

Abstract

Recent advances in the endovenous surgery, such as saphenous vein ablation, have broadened our understanding of pathophysiology and treatment strategy in patients with chronic venous insufficiency. These advances lead the emerging of a minimally invasive management of incompetent perforating vein (IPV). It is known as percutaneous ablation of perforators (PAP). All methods of PAP have similar commonalities such as ultrasound-guided percutaneous intraluminal access; application of some energy source within the lumen leading to vein wall contraction and/or occlusion; treatment in an outpatient office setting; simple retreatment, if needed. This article reviews the treatment of the pathologic perforators.
